I worked as a Case Manager for many years. Case Management doesn’t have a universal definition. One organization describes case management as a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet an individual’s needs. Generally speaking, case management refers to the coordination of services and resources on behalf of an individual. I assisted clients with everything from housing to budgeting to employment.
